The problem we solve: The scope of U.S. health problems is jaw-dropping, Chronic illness, mental health & addiction; nearly 70% of the U.S. population struggles daily with 1 or more of these issues and it accounts for 75% of healthcare spending
The number of people suffering from these health issues continues to rise, but the number of providers is slowly declining. Add in the current Opioid Crisis & the result is an overworked & understaffed national health system.
About our solution:
KnKt'd Health brings treatment into patient's everyday lives, using a progressively triggered technology solution (patient app/encrypted cloud database/provider website portal) that enables patients and providers to work collaboratively towards better places of wellness. HIPAA complaint, encrypted and based from current evidenced based practices. With our lego block platform design we are continually able to show that KnKt'd is infinitely scalable and easy to use.

About Our Team
Creator: Matt Holland

Bio: Matt Holland has been working in the behavioral health field collectively for over 18 plus years in 3 different States in the western United States. After many years working on the ground level, supervising and administrating programs across many health systems, he saw too many huge disparities in the systems he worked in and for. After a time, the dream of KnKt’d was born.
Matt continues to advocate for change in the collective health system on various councils and committees. He wrote on the Pulse and continues to write on Huffington Post. Talks frequently on panels and during speaking engagements about the changing landscape of behavioral health and the new era of technology.
In his free time, he enjoys reading, hiking, spending time with his family and enjoying the world around him.

KnKt’d Health was created as a result of first-hand, working knowledge of how difficult it is to disseminate information in the healthcare field. Patients/consumers have limited access to information and healthcare professionals, whether it be physical or behavioral healthcare, struggle to keep up with ever increasing numbers of persons who come to them for services.
The KnKt’d app provides the opportunity for healthcare professionals to monitor their patients/ consumers in between appointments with a visual patient dashboard which is color coded for prioritization and alerts based on patient/consumer data entry. It takes 3 to 5 seconds for information to reach providers once a patient/consumer has entered information into the KnKt’d app on the patient side. Patient responses are displayed on the provider portal which allows the provider to see immediately which patients are trying to communicate, or are in distress, and, in an instant, the provider can pull up the specific person’s data showing detailed information in longitudinal response patterns.
The provider portal allows specific modules to be uploaded based on the individual consumer’s needs and will track level and type of pain, mood, sleep, medication compliance, treatment goals and skills/support utilization; it will also send appointment reminders and provider-directed journaling assignments. The app allows for secure messaging between the patient and the provider, tracks treatment engagement and offers the option to provide additional resources for wellness.
On the patient/consumer side, access through the app is simple. It can be accessed via smartphone or computer. Push notifications and progressively triggered daily questions in simple, plain language guide the patient through the process. Their answers are converted into useable data via our proprietary algorithms, graded on a Likert Scale and then color coded for display on both the patient app and the provider portal, allowing access to this information in real time.

KnKt’d Health is built by healthcare professionals turned technologists with both ground-level service and high-level administrative experience. We know the frustration of too many patients and not enough time. We are also keenly aware of the need for a technology that can keep up with the demands of today’s healthcare system, and, to change how services are provided so people are willing to stay engaged, and can afford to stay connected to their healthcare providers.
This is why we built our platform to be infinitely scalable, focusing on patient care, experience and engagement. KnKt’d Health creates linkages to prevention and health promotion services, information and educational resource dissemination, local resources and support, as well as robust inbound client-specific data which can be monitored and addressed in real time. The use of our technology within a fusion housing model has already demonstrated a sustained 60-80% reduction in the costs of acute emergency and hospital care for the highest utilizers (high needs medical and psychiatric health conditions along with addictions) in that catchment area.

The KnKt'd team has not built one monolithic system that will be resistant to updates once installed. That is the traditional path in health care information technology systems across the globe. KnKt'd has created a system built with Lego-like components that can be highly configured based on needs; this is the key our success. Allowing us to expand KnKt'd based on population need/use and the ability to quickly adjust modules to improve them.
