HealthPasskey is a web based application that integrates patients in their health wellness by providing innovative tools while coordinating care between healthcare organizations.
Our novel solution allows patients to access their clinical information and track wellness plans, securely exchanges information with other healthcare organizations, and helps reducing hospitals readmission rates by providing a comprehensive discharge follow up plan.
The objective of HealthPasskey is to provide innovative and patient engaging tools, coordinate care between healthcare organizations, and reduce healthcare costs while keeping patients healthy.
How it Works
HealthPasskey provides access to three secured portals: Patient, Provider & Organization. Using these it coordinates care between patients, physicians, and healthcare organizations.
HealthPasskey’s Patient Portal assists patients in:
- Obtaining information about physicians before they schedule an appointment
- Booking appointments with physicians on their preferred date and time
- Receiving reminders via text message and email for booked appointments the week before, the day before, and the day of the appointment
- Receiving text messages and emails for recommended tests or evaluations based on health conditions, age, sex, & other factors
- Accessing their complete record of lab results, doctor’s visits, clinical and medication history, allergies, etc
- Assessing the physicians they visited by eSurvey prompts after each appointment
- These features and much more are FREE of charge!
HealthPasskey’s Provider Portal also creates a strong patient-provider relationship, while connecting providers to other healthcare practices and centralizing patient information and services. Providers can:
- Coordinate care with other healthcare settings
- Enhance patient-provider communication
- Manage and share patients’ clinical information
- Configure their schedules by their working days and time, by appointment reason, or duration per visit
- Practice preventive medicine by providing patients with instructions to be followed for the appointment reason selected and information about diseases
- Access their schedules and patients’ clinical information from anywhere in the world
- Make referrals to colleagues
- Obtain statistical reports
The HealthPasskey’s Organization Portal allows hospitals to:
- Become a virtual hospital
- Schedule discharge follow-up appointments with other caregivers
- Reduce readmission rates for Acute Myocardial Infarction, Heart Failure, and Pneumonia
- Avoid the Readmission Reduction Program penalties beginning in October 2012
Clinical Proof
The Centers for Medicare & Medicaid Services (CMS) will begin penalizing hospitals with elevated Acute Myocardial Infarction (AMI), Heart Failure (HF), and Pneumonia (PN) readmission rates in October 2012.

How We Help Patients
- Patients do not have access to all of their clinical information. HealthPasskey provides patients access to their own clinical history, appointment history, drug history, allergies, and other information from each visited healthcare provider.
- The average waiting time at doctor’s offices in the United States is 24 minutes; with HealthPasskey, there is no more need to wait! Through HealthPasskey, doctors can let patients know of their status via text messages and emails. This means that if a doctor is running late, the patient will know and proper arrangements can be made.
- Sometimes patients forget about their doctor’s appointments. With HealthPasskey, no more missing doctor’s appointments! HealthPasskey will send you appointment reminders the week before, the day before, and the day of the appointment via text message and emails.
- It is very difficult now to access our dependant’s clinical information, but HealthPasskey has a solution for that. HealthPasskey gives patients the opportunity to add dependents to their accounts and have access to dependant’s clinical information.
- Patients typically do not have enough information about physicians. HealthPasskey gives, in the providers’ catalog page, information about physicians such as contact information, schedules, location map, and other patients’ ratings. This innovation allows patients to choose the physician that best suits their needs.
How We Help Doctors
- Healthcare settings are not connected with each other, resulting in a lack of coordination of care. HealthPasskey creates a secure connection among healthcare settings where patients’ clinical information is securely exchanged. The secure connection provides important benefits for physicians:
- Physicians are able to make better informed decisions about their patient’s condition, since HealthPasskey gives them a broader picture of the patient’s clinical history.
- Healthcare settings are able to securely exchange information with other healthcare organizations, setting them up as meaningful users under the ARRA act.
- Physicians need robust communication with patients. HealthPasskey enhances patient-physician communication by:
- Allowing patients to share clinical information available in their personal health records with physicians
- Providing physicians with tools that make instructions available to patients for them to follow before & after appointments and procedures.
- HealthPasskey also gives physicians the exposure to drastically increase their client base and reputation
- Patients from all over the country can see a physicians’ basic information on the Providers’ Catalog page and learn more about the special procedures or services he/she provides.
- HealthPasskey allows physicians to reach more patients. The application is available in English & Spanish, and is ready to be deployed in any other language.
How We Help Hospitals
- On October 1st, 2012, CMS will begin to penalize hospitals with high readmission rates for Acute Myocardial Infarction (AMI), Heart Failure (HF), and Pneumonia (PN). Through the Organization Portal, hospitals’ caregivers, administrators, and other personnel with suitable credentials can book follow-up appointments with other providers for patients being discharged. In addition to booking follow-up appointments with their colleagues, providers can schedule transportation services the day of the appointment for the patient if needed.
This substantially reduces hospitals’ re-admission rates due to ensured follow-up treatment post-discharge. This software has the power to liberate hospitals from being penalized in the upcoming years.
- We have developed a pilot program strategy to implement HealthPasskey in hospitals to: significantly reduce readmission rates for AMI, HF, and PN; stimulate provider and patients engagement; implement data-mining features; establish state of the art analytics; & strengthen patient-providers, providers-organization, and providers-providers inter-connectivity.

Your contributions will be fully utilized to develop the first iteration of HePak Wellness Plan. HePak Wellness Plan will connect doctors, nutritionists, chefs, and physical trainers to control patients’ weight applying a scientific approach. HePak Wellness Plan will revolutionize the industry by integrating patients’ medical conditions to the weight plan! Here are some of its features:
- Snacks, exercise, and medication reminders to patients via text messages and email

Who We Are
- Luis Figueroa is the CEO of the company and the creator of HealthPasskey™, HePak WS™, and HePak NHIN Node™. He has more than 20 years of experience in software development and in the health informatics industry.
- Luissette Figueroa is the Vice President of the company. She is a software engineer with a master’s degree in Engineering Management and soon to culminate her second master’s degree in Health Informatics and Information Management.
