NONNATECH – Digital Health Behavioral Monitoring Solution

Overview:

This the first digital health solution providing real-time data showing changes in behavior (or the absence of behaviors) that signal physiological changes in patients being monitored. The early detection and intervention for physiological changes allows clinicians to more effectively prevent older adults from being admitted to hospitals and higher levels of care and allows older adults to remain living in their communities for longer periods of time. This system allows Family Members, Senior Living Communities, Health Plans, and Managed Care Organizations to impact and participate in monitoring the healthcare of the functionally impaired and chronically ill population.

%{color:blue}*The Problem We Address:

The chance of having a stroke approximately doubles for each decade of life after age 55. According to the US Census Bureau, the population of individuals aged 65 and over is projected to reach almost 64 million by 2025. The total value of services and supports for seniors in the United States, including the estimated economic value of informal (or donated) care, exceeded $400 billion as recently estimated by the Congressional Budget Office.

According to the National Alliance for Caregiving and the AARP, more than 65 million people, 29% of the U.S. population, provide care for a chronically ill, disabled or aged family member or friend during any given year and they spend at least 20 hours per week providing care for these loved ones. 13% of family caregivers are providing 40 hours of care a week or more. Now Nonnatech has the latest technology to provide real-time monitoring of all patients and automatic data transfer to clinicians, family members, and home care staff which is passively collected.

According to a report by the Healthcare Costs and Utilization Project, approximately 75% of hospitalized patients are able to return to their home following discharge. Discharge to a long-term care facility was the second most common type of discharge which accounted for 12% of such discharges. Discharge to the home with additional home health care supervision accounted for 9%. The remaining discharge arraignments each accounted for 2% or less of discharges. According to the federal government, 20% of elderly patients are back in the hospital within 30 days after being discharged. According to a recent study conducted, remote monitoring allowed elderly and chronically ill individuals to experience over 35% fewer emergency department visits, 14% fewer days in long-term care, and 7% fewer inpatient admissions as compared to the control group.

*About Our Solution: *

Nonnatech enables healthcare professionals and family members to intervene early on, thus preventing and managing potentially deteriorating conditions more quickly. This solution often contributes to preventing hospitalizations and (re)admissions into higher levels of care. Nonnatech provides a pathway to lower level interventions (containing costs), preventing long term and acute care (re)admissions and reduces length of stay in rehabilitation facilities. Our platform monitors behavioral changes with real-time data collected from our connected health devices. These behavioral changes can reflect medication adherence (or lack thereof) and significant activity levels in regard to eating, fluid intake, sleeping, toileting, and socialization patterns. Our solutions provide clinicians with valuable real-time data (for continuous monitoring of patients with chronic conditions such as congestive heart failure, cardiac arrhythmias, diabetes, and pulmonary diseases) through the use of our unique monitoring devices and predictive analytics. Therefore, clinicians will be able to monitor and assess significant physiological changes and identify potential deleterious changes in patient health. Nonnatech’s solution produces overall cost savings for the healthcare system. Through the use of this technology, it is possible for healthcare professionals to more efficiently care for patients, results in less stress for professionals and family members and it allows for a higher caseload per clinician.

Progress to Date:

Nonnatech is a 2014 PILOT Health Tech winner and has been awarded $100,000 (on a nearly $250,000 operating budget) to implement an innovative health solution project grant in the Bronx, NY. The award has allowed Nonnatech to partner with the Hebrew Home at Riverdale (which cares for over 10,000 seniors throughout New York daily). The Hebrew Home at Riverdale provides a complete range of services and programs for older adults which includes managed long term care, home care, assisted living, low and middle income senior apartments, post-acute rehabilitation, community programs and services. It also houses the nation’s first elder abuse shelter. The following link discusses Nonnatech as winners of an innovative project grant by PILOT Health Tech NYC: EDC Press Release

In addition, Nonnatech is currently working with several additional Managed Care Organizations, Hospitals, Health Plans, Skilled Nursing Facilities, Senior Living Communities, large Fortune 500 companies, and many more partners to provide digital health remote monitoring in underserved populations throughout the world.
Nonnatech is one of the few digital healthcare solutions in the United States to have participated on the SmartAmerica Closed Loop HealthCare team launched by the White House Presidential Innovation Fellows and the National Institute of Standards and Technology (NIST) in 2014. The team was able to demonstrate how data can move seamlessly from home to ambulance to hospital and back home, thus producing overall cost savings and thus improving health outcomes.

How Our Solution Aids in Prevention of Heart Disease:

Nonnatech is the ONLY Digital Health Solution that has integrated all of the following devices on to our platform in order to provide passively collected real-time data and automatic data transfer to clinicians, family members, and home care staff:
• Bed Sensors
• Toilet Sensors
• Incontinence Sensors
• Chair Sensors
• Floor Sensors
• Nonna Buttons
• Motion Sensors
• Lamp/Appliance Controllers
• Temperature Sensors
• Open/Shut Sensors
• Water Sensors
• And much more

Here is what that looks like:

Nonnatech’s Sensors and Other Devices in a Senior Living Community

Nonnatech’s Sensors and Other Devices in an individual’s Kitchen

Unobtrusive sensors are placed anywhere throughout the home, in senior living communities, in hospitals or in medical facilities. Our technology works on both a cellular network and Wi-Fi connection.

Prevention and better management of heart disease and stroke can be accomplished by using Nonnatech’s solution to continuously monitor patients with functional impairments and chronic conditions such as congestive heart failure, cardiac arrhythmias, diabetes, and pulmonary diseases. Lowering blood cholesterol and blood pressure can be accomplished by assuring adherence to the patient’s medication schedule. Our platform is able to monitor whether patients are exercising on a daily basis and changing a possible sedentary lifestyle. This is accomplished through measuring/monitoring of behaviors (or the absence of behaviors) which reflect medication adherence (or lack thereof) and which indicate significant activity levels regarding eating, fluid intake, sleeping, toileting, and socialization patterns. Healthcare professionals and family members can be alerted to initiate early interventions which may thus prevent/effectively manage potentially deleterious conditions. Additionally, some scientists have noted a relationship between coronary heart disease risk and stress in a person’s life. Stress may affect the risk factors for heart disease and stroke. Nonnatech’s real-time monitoring platform lessens stress for patients, clinicians, and caregivers. According to the National Alliance for Caregiving, 72% of caregivers report not going to the doctor as often as they should and 63% of caregivers report having poorer eating habits than non-caregivers before assuming caregiving responsibilities. This allows more time for caregivers to participate in healthy activities and results in a better lifestyle.

Nonnatech Benefits Many Stakeholders in Healthcare

Benefits to Patients and their families

Our solution provides better patient care and the ability for individuals to remain in their communities for longer periods of time. Nonnatech’s solution enables family members to contain costs and reduce ER visits, hospital admissions, and nursing home placements. Along with the user retaining their dignity and privacy, family members are able to receive real-time notifications if there is an emergency event which requires immediate attention or if their loved is at risk for such an event. Our affordable, easy to use system, is a great way for family caregivers to stay in touch which enables family engagement and monitoring of their loved one from anywhere in the world at any time.

Benefits to Hospitals, Institutions, and/or Medical Facilities

Nonnatech’s solution creates staffing efficiencies by providing bedside nurses, aides, and nurse managers with critical real-time data regarding the number of patient falls and other metrics which the healthcare organization is interested in tracking through their service units. Not only will our platform help monitor the number of accidents and falls, but through utilizing our unique monitoring devices and predictive analytics, clinicians will be able to assess significant physiological changes and reduce/prevent accidents and falls plus numerous potential deleterious changes in patient health. This solution helps make the lives of professionals easier, more efficient, and thus increases accuracy in clinical decision making. Using Nonnatech’s predictive analytics through measurement of behavior, valuable data is gathered on a real-time basis post-discharge in the patient’s residence which helps to reduce unnecessary readmissions which can then generate significant monthly savings and return on investment (ROI).

Benefits to Health Plans and Managed Care Organizations

With Nonnatech’s digital health solution, Nonnatech produces significant monthly savings and increases ROI for healthcare payers through a reduction in unnecessary emergency department visits, hospitalizations, and nursing home placements. This results in better case management and allows for a higher caseload per clinician. Nonnatech’s technology is able to assist community health workers improve care coordination to patients discharged from hospitals, rehabilitation facilities, and inpatient or outpatient clinics. The platform will allow the community health workers to obtain critical real-time data (for continuous monitoring of potential health risks such as urinary tract infections, sleep apnea, depression, dehydration, etc.) and this provides for a vital continuity of care once the patient is discharged/released. This continuous monitoring will reduce readmission rates.

How We Help IoT Device Makers

Consumer health and medical device manufacturers and service providers have approached Nonnatech to integrate weight scales, blood pressure monitors, glucometers, pulse oximeters, and more medical devices. This would enhance the ability to monitor blood sugar levels, blood pressure, and cholesterol, helping to further lower chances of having a heart attack and stroke. We are open to many such relationships and can provide our time and API kit to connect. Please see the PARTNER options at the right for a few options. To explore more please contact us.

How We Further the American Heart Association’s 2020 Mission

Nonnatech’s technology was created to support the elderly and other vulnerable populations with high target disease prevalence and early to moderate cognitive impairments who are at risk for cardiovascular disease, diabetes, stroke, and additional chronic health conditions. Such issues may contribute to long term and acute care (re)admissions, longer stays in rehabilitation facilities or even death. Hospitalizations, specifically in older adults, may result from a general decline in function, poor quality of sleep, exacerbation of a chronic illness such as Alzheimer’s Disease or an underlying acute illness such as an urinary tract infection. Polypharmacy issues that result from individuals aged 65 and older taking numerous combination of medications can also contribute to hospitalizations, heart attacks, strokes, and even deaths. Nonnatech’s digital health solution is able to reduce unnecessary emergency department visits, hospitalizations, and readmissions. According to a recent study conducted, remote monitoring allowed elderly and chronically ill individuals to experience over 35% fewer emergency department visits, 14% fewer days in long-term care, and 7% fewer inpatient admissions as compared to the control group.

How We Will Use the Proceeds of Funding:

Nonnatech has a developed product which will be enhanced during the coming year of operation. Additional funds will go for further development of our platform and system (including the improvement of current designs for devices and software). The raising of additional money would for further growth and development of our products. It would allow for major growth and enhance our ability to reach a larger market. Additional funds would allow us to work with additional strategic partners interested in integrating their technology with our revolutionary digital health solution.
